About The Position

The Home Health Aide (HHA) participates in direct patient care and other supportive personal care assistance to patients as outlined by the patient’s individual plan of care and under the direction of licensed nursing staff. Essential Responsibilities: Responsibilities listed in this section are core to the position. Inability to perform these responsibilities with or without an accommodation may result in disqualification from the position. Perform acts that are within the scope of practice as defined by the applicable certifying board. Demonstrate the knowledge and skills necessary to provide care to patients as described in unit/department-based competencies, policies, and procedures. Accurately evaluate patient needs and develop/implement/review/maintain a comprehensive care plan. Evaluate and provide care as directed to achieve desired patient outcomes for patients newborn thru geriatric. Understanding, observing, and reporting to the hospice team in a timely manner; the patient/family physical, emotional and psychosocial aspects of care, disease process, symptom and pain management, and signs and symptoms of death (when appropriate). Complete timely and accurate documentation as per Health System/affiliate policy. Act in accordance with the established mission, vision, and values. Abide by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI). Maintain effective communication and professional working relationships with patients/clients and their representatives, team members, contractors, physicians, peers, outside agencies, and the public. Minimum Qualifications: Required Education and Experience Successful completion of an approved Home Health Aide (HHA) program in the state of Kansas. Required License/Certifications/Registrations Current certification as a Home Health Aide (HHA) in the state of Kansas. If certified as CNA, HHA is required within 3 months of hire. Current BLS/CPR card from approved provider within 30 days of hire or by the end of orientation, whichever comes first. Kansas Driver’s License (if position requires driving) Auto Insurance equal to or greater than the Kansas Insurance requirements (if position requires driving) Preferred Qualifications: Preferred Education and Experience Previous CNA/HHA Previous care of patients at end of life Preferred License/Certifications/Registrations CNA Knowledge, Skills, and Abilities: Basic computer skills. Multi-tasking and prioritization skills. Demonstrated ability to communicate effectively. Ability to apply the principles of patient care. Physical Requirements: With or without accommodation. Heavy Work: Occasionally exerting up to 100 lbs - constantly exerting up to 20 lbs. 51-75% of the day may be standing or walking. Requirements

  • Successful completion of an approved Home Health Aide (HHA) program in the state of Kansas.
  • Current certification as a Home Health Aide (HHA) in the state of Kansas. If certified as CNA, HHA is required within 3 months of hire.
  • Current BLS/CPR card from approved provider within 30 days of hire or by the end of orientation, whichever comes first.
  • Kansas Driver’s License (if position requires driving)
  • Auto Insurance equal to or greater than the Kansas Insurance requirements (if position requires driving)
