What You'll Do: Works under the direction of the Home Health Coordinators and reports to the Home Health Manager. Responsibilities include but not limited to providing participants assistance with personal care and household management in the home setting. Assist participants with basic activities of daily living including but not limited to bathing, dressing, grooming, incontinent care, toileting, etc. as assigned. Assist participant with transfers and mobility with or without assistive devices. Maintain a safe, clean and healthy environment through light housekeeping, including changing bed linens, dusting, sweeping, cleaning kitchen and bathroom, and doing laundry. Provide companionship and stimulation for the participant. Accompany the participant to doctors or other appointments as needed. Perform other housekeeping tasks as indicated in the Care Plan. Remind participants to take medication at the appropriate time. Plan and prepare simple meals; serve meals, supplements and snacks; and cleanup. Assist with oral feeding of the participant if needed. Observe/report changes in the participant's condition to Home Health Coordinator of Manager. Participate in training and in-service education programs. Perform participant transfers regularly with the use of mechanical devices as indicated. Travel to participants; homes and visit multiple participants on the same day (4-5 participants) Work with participants with varying cognitive levels, physical and sensory impairments, limited social supports, declining health and at end-of-life. What You'll Bring: High School Diploma/GED and CPR certification preferred. Current Certified Nursing Assistant License preferred. Minimum two-years related experience with older adults required. Strong interpersonal skills with the ability to handle unusual situations.
