Midland Care Connection Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas. Visting Nurse Association Specializes in home hospice that provides compassionate, end-of-life care to patients in the comfort of their own homes. Their skilled nurses focus on managing pain and symptoms, supporting emotional and spiritual needs, and guiding both patients and families through the hospice journey. Working closely with an interdisciplinary team, the VNA ensures dignity, comfort, and quality of life during a patient’s final days while offering education, resources, and respite support to caregivers. Home Health Aide Provides hands-on personal care and comfort support to patients receiving end-of-life care at home. Their role focuses on maintaining dignity, comfort, and quality of life while also giving family caregivers practical support and relief.
