About The Position

A Home Health Aide (HHA) provides direct patient care to patients under the direction of the RN and according to the Aide Plan of Care (POC). The HHA assists patients with self-administered medications, takes accurate vital signs, helps maintain personal hygiene, assists with feeding, toileting, mobility, and documents care provided. The role also includes practicing infection control principles and assisting with household duties when necessary.

Requirements

• High school diploma or equivalent.
• Completion of Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
• Must meet applicable state certification requirements.
• A valid driver's license, auto insurance, and reliable transportation.
• Must be in good standing on the HHA Registry (if applicable).
• At least one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant.

Responsibilities

• Provides direct patient care under the direction of the RN and according to the Aide Plan of Care (POC).
• Assists the patient with self-administered medications.
• Takes accurate temperature, pulse, and blood pressure measurements.
• Helps patients maintain good personal hygiene.
• Assists in feeding patients and communicates basic principles of nutrition.
• Assists with patient toileting and catheter bag changes.
• Provides skills to assist with patient mobility, exercises, and transfers.
• Documents interactions with patients, caregivers, and other staff members.
• Practices acceptable infection control principles.
• Assists with household duties including light laundry and meal preparation.
