About The Position

A Home Health Aide (HHA) provides direct patient care to patients under the direction of the RN and according to the Aide Plan of Care (POC). The HHA assists patients with self-administered medications, takes vital signs, helps maintain personal hygiene, assists with feeding, and provides support for toileting and mobility. The role also includes documenting patient interactions and ensuring a clean and safe environment. Additional household duties may be required as needed.

Requirements

• High school diploma or equivalent.
• Completion of Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
• Must meet applicable state certification requirements.
• A valid driver's license, auto insurance, and reliable transportation.
• Good standing on the HHA Registry (if applicable).
• At least one year of experience as a Home Health Aide or Certified Nursing Assistant in the last 24 months.

Responsibilities

• Provides direct patient care under the direction of the RN.
• Assists patients with self-administered medications.
• Takes accurate temperature, pulse, and blood pressure measurements.
• Helps patients maintain personal hygiene.
• Assists in feeding patients and observes food and fluid intake.
• Assists with patient toileting and catheter bag changes.
• Supports patient mobility, exercises, and transfers.
• Documents patient interactions and care rendered.
• Practices acceptable infection control principles.
• Assists with light household duties as needed.
