About The Position

The Certified Home Health Aide (CHHA) plays a crucial role in the Home Health Team by providing direct patient care under the supervision of a case manager. This position is responsible for assisting patients with daily living activities, reporting changes in their condition, and ensuring that patients receive the necessary support to maintain their health and well-being at home.

Requirements

  • Successful completion of a training program approved by the Department of Health Services.
  • High School Diploma/GED; California Home Health Aide Certificate.
  • Current BLS certification; California Driver's License.

Nice To Haves

  • Computer literate preferred.
  • Experience in automated clinical information systems preferred.

Responsibilities

  • Assisting patients with personal hygiene such as skin, mouth, hair care, and bathing.
  • Assisting patients in and out of bed and with ambulation.
  • Assisting with prescribed exercises taught by health personnel.
  • Preparing meals, including therapeutic diets, and assisting patients with eating.
  • Assisting patients to the bathroom or in using commodes, bedpans, or urinals.
  • Performing household services to facilitate patients' self-care at home.
  • Assisting patients with medications that are ordinarily self-administered.
  • Performing activities taught by health professionals, such as changing colostomy bags and taking vital signs.
  • Reporting changes in patients' conditions to the supervising nurse or therapist.
  • Completing records regarding services performed.
  • Taking actions in emergent patient situations and reporting to the supervisor.
  • Preparing timely and complete documentation of all care provided.
  • Documenting all communication with other providers.
  • Reporting changes in patient status to the case manager.
  • Submitting schedule logs and daily progress notes within expected time frames.
  • Participating in care coordination meetings.
  • Organizing, planning, setting priorities, and completing assignments within agency guidelines.
  • Completing expense reports, time cards, and schedule logs.
  • Behaving in a courteous and professional manner with patients and health professionals.
