We are looking for an experienced, talented, and compassionate Home Health Aide Clinical Manager for a full-time leadership position supporting both our Home Health and Hospice programs. The Full-Time Home Health Aide Clinical Manager is responsible for the clinical oversight, coordination, and operational alignment of our Home Health Aide workforce. The Clinical Manager will ensure safe, high-quality, and compliant delivery of aide services while partnering closely with Intake, Clinical Managers, and Staffing to support timely admissions and continuity of care.

Requirements

  • Must be a Nurse, OT or PT and have a valid license in MA
  • Medicare-certified Home Health and/or Hospice experience required
  • Knowledge of Home Health Aide supervision requirements and Conditions of Participation
  • Minimum of 2–3 years of clinical experience
  • Experience working with electronic medical records

Nice To Haves

  • Prior leadership or supervisory experience preferred

Responsibilities

  • Supervise and provide clinical oversight of Home Health Aide services across Home Health and Hospice programs.
  • Oversee workforce scheduling, deployment, and alignment with referral demand.
  • Coordinate required supervisory visits in accordance with Medicare Conditions of Participation.
  • Collaborate with Intake and Clinical leadership to support timely admissions and continuity of care.
  • Lead hiring, onboarding, training, competency validation, and performance management of Home Health Aides.
  • Ensure regulatory compliance with state and federal standards.
  • Promote quality improvement initiatives and maintain survey readiness.
  • Support interdisciplinary collaboration to enhance patient outcomes and satisfaction.
  • Contribute to operational planning and workforce development strategies.
