About The Position

This position involves collaborating with interdisciplinary teams to deliver quality patient care under the direction of a registered nurse. The role includes assisting in performing personal care needs and providing compassionate end-of-life care. The position will float to four hospice home locations and requires weekend coverage as needed.

Requirements

  • California Home Health Aide (HHA) certification - CA Department of Public Health - REQUIRED
  • California Certified Nurse Assistant (CNA) certification - CA Department of Public Health - REQUIRED
  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) certification - American Heart Association - REQUIRED
  • Driver's License - CA Department of Motor Vehicles - REQUIRED
  • ServSafe Food Handler Certified - National Restaurant Association - REQUIRED

Nice To Haves

  • 1 Year experience in acute care, home health, or hospice.
  • Current Food Handler card or acquisition within 90 days of hire.
  • Utilizes reliable transportation and possesses adequate personal insurance coverage.
  • Demonstrates clean driving record in accordance with requirements of the employer DMV pull notice program.

Responsibilities

  • Collaborate with interdisciplinary teams in the delivery of quality patient care.
  • Assist in performing personal care needs and compassionate end-of-life care.
  • Contribute to overall unit functions.
  • Observe patient condition and report changes to the RN.
  • Report pertinent patient-related information to RN.
  • Communicate effectively and participate in the coordination of the plan of care.
  • Perform routine equipment maintenance according to departmental policy.
  • Maintain all certifications required of the job.
  • Provide comprehensive personal care to patients under the supervision of the RN.
  • Maintain privacy and confidentiality at all times.
  • Demonstrate and maintain expertise through attendance and participation in department in-services and meetings.
  • Maintain a safe working environment for patients, families, and other team members.
