About The Position

HOME HEALTH HOME SUPPORT AIDE (HHA) - PER DIEM Position Summary For over 40 years, North Hill, a not-for-profit senior living community located in Needham, Massachusetts, has provided vibrant living opportunities to people 65+ by offering a host of hospitality, wellness and health care services. The Home Health Aide who will provide care for patients and administer necessary services and functions in compliance with Medicare Condition of Participation, State and Federal Regulations, North Hill’s policies, procedures, and standards. The individual will partner with patients to provide patient centered care including Activities of Daily Living and light homemaking activities. Job Specific Duties Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Provides patient centered care including activities of daily living : bathing, dressing, grooming, nail care, shaving, and bowel and bladder functions Follows a written plan of care when providing direct patient care and ensures that training has been conducted by the Registered Nurse Completes required visit documentation and communication notes within 24 hours of home visit Complies with orientation and supervisory visit schedules Reports all changes in patients condition to the nurse and completes necessary documentation in a timely manner Assists with responding to all emergencies Advocates for and understands patient personal preferences and provides support/care in a manner that respects patient rights, choices, confidentiality, and privacy and protects them from abuse Completes necessary documentation for ancillary services and supplies to ensure tracking and correct billing can occur. Utilizes the electronic health documentation appropriately Completes refrigerator temperature check log as assigned Participates in interdisciplinary team meetings and provides input for care coordination and collaboration Adheres to infection control protocols and attendance at Quality Assurance and Performance Improvement meetings Completes required 12-hours of in-service education annually per Medicare Conditions of Participation Safety requirements and exposure procedures Follow all Emergency Management Plan and actively participates in fire and evacuation drills Meets department uniform, appearance and grooming requirements Complies with all OSHA safety regulations, local applicable laws regarding health, safety or environment North Hill is committed to providing equal opportunity for employment and advancement to all team members and potential team members. Requirements

  • Ability to maintain confidentiality and strong customer support skills
  • Complies with protection of health information (HIPAA) safeguards
  • Ability to work independently with limited supervision
  • Work professionally with people from a wide variety of cultural and socio-economic backgrounds
  • Must possess the ability to deal tactfully with Team Members, Residents, family members, visitors, and the general public
  • When working in Resident areas, must be able to communicate effectively in English
  • Attends and participates in in-service educational classes, Healthcare Academy and staff meetings as directed
  • High School Diploma or equivalent
  • Certified Home Health Aide certification or Certified Nursing Assistant in the State of Massachusetts
  • Ability to read, write and understand English
  • 2 years’ experience as a Certified Nursing Assistant or Certified Home Health Aide, preferably in a home health care setting
  • Proficient with computer software programs
  • Satisfactory completion of a background investigation is required for this position.

Responsibilities
