About The Position

Hilton East Assisted Living currently has multiple Full-time/part-time openings for a Home Health Aides/PCA level 2 at our assisted living facility located in Hilton, NY. Requirements

Education: High school diploma or GED.
Licensure/Certification: Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification in New York State.
CPR and First Aid certification (or willingness to obtain).
Experience: Prior experience in an assisted living, nursing home, or home care setting preferred.
Experience in administering medications or assisting with medication management preferred.
Skills: Ability to work independently and as part of a team.
Strong organizational skills and attention to detail, especially in documentation and medication administration.
Excellent communication skills with residents, families, and the care team.
Ability to lift, move, and assist residents in a safe and respectful manner.
Ability to stand, walk, bend, kneel, and perform physical tasks throughout the workday.
Must be able to handle insulin checks/injections.

Nice To Haves

Experience in an assisted living or healthcare setting.
Familiarity with electronic health records (EHR) or medication management systems.

Responsibilities

Works under the supervision of the Registered Nurse and/or Licensed Practical Nurse in accordance with the New York State Department of Health regulations.
Provides clients and/or residents with assistance with ADL’s and housekeeping tasks, as dictated by the Nurse and/or Patient Care Plan
Able to follow the Nursing Care Plan to complete the daily activities of the client and/or resident, as well as completion of the aide activity sheet.
Complete all documentation in a neat and timely matter
Observe the client and/or resident, and report changes appropriately to RN or LPN
Attend required In-service training and HHA meetings. Must complete 12 In services per year to stay certified.
Performs tasks and duties assigned or requested by shift supervisor.
Exhibits necessary interpersonal skills to work well with clients and families.
Work hours as scheduled by supervisor.
