About The Position

Home Care CNA or HHA - Flex 2 Per Diem Program Must have a valid Connecticut Driver's License Per Diem positions where employees commit to work 2 shifts every other weekend Shifts will be assigned based on needs of the organization and can range from 4 to 12 hours per shift. Additional shifts may be worked. These shifts will be assigned according to the needs of the organization. In the event the facility requests additional coverage, any time over forty hours worked will be paid at time and one-half. Minimum Qualifications : Education: High School Diploma or GED Experience: 1 year experience as a Certified Nursing Assistant or Home Health Aide Licensure: CNA or HHA Certification and Valid CT Driver's License Job Standards: Under the clinical direction of the Nurse Case Manager and administrative direction of Coordinator, Home Care, provide direct patient care services as outlined in essential job function. Required to lift, transfer, and transport adult clients. Provide personal care to clients and keep records of personal health care activities as defined by regulatory requirements. Dementia patient experience REQUIRED.

Requirements

• High School Diploma or GED

• Dementia patient experience

Responsibilities

• Provides direct patient care services.
• Required to lift, transfer, and transport adult clients.
• Provide personal care to clients and keeps record on personal health care activities as defined within the respective regulatory environment.
• Requires working some flexible hours.
