The Home Health Aide is responsible for providing personal care and may at times provide meal preparation or light housekeeping to a designated group of patients as directed by a Registered Professional Nurse, Physical Therapist, Occupational Therapist or Speech Language Pathologist. Provides routine individualized healthcare such as changing bandages and dressing wounds, applying topical medications to the elderly, convalescents, or persons with disabilities at the patient's home or in a care facility. Assuming appropriate training, may also perform lab work, and other patient care related tasks. The incumbent is responsible for para-professional aspects of care, patient safety and may include installation of telehealth equipment in the patient's residence as written in the patient's individualized plan of care. The Home Health Aide monitors or reports changes in health status and collaborates with the patient's primary clinician and other members of the patient care team.
