Requirements
- Active CHHA certification
- High School Diploma or GED
- Minimum 1 year of direct patient care experience (home health preferred)
- Reliable transportation and valid driver’s license
- Self-motivated with a passion for helping others
Responsibilities
- Provide high-quality, compassionate in-home care
- Assist patients with daily activities and personal care
- Maintain accurate documentation of care provided
- Communicate effectively with the care team and families
- Travel daily to patient homes (reliable transportation required)
