About The Position

The Visiting Nurse Association is seeking a compassionate and organized Admissions Nurse to join our dedicated healthcare team. In this vital role, you will be responsible for performing initial, in-home assessments, collaborating with physicians to determine eligibility, and developing a comprehensive, personalized care plans for patients. This role acts as the critical first point of contact, ensuring a smooth transition into home care. Requirements

  • Valid Registered Nurse (RN) license in the state of Iowa
  • A graduate of an approved school of nursing (RN/BSN/ARNP) with a current Iowa Nursing License.
  • Two years of home health or hospice or work related experience preferred. General medical knowledge related to home health and hospice services with end-of-life issues is desirable. Strong OASIS documentation skills preferred.
  • Reliable car, driver's license and adequate insurance as outlined in the personnel policies is required.
  • Strong clinical knowledge, critical thinking skills, and evidence-based practice expertise.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to prioritize tasks, delegate responsibilities, and manage multiple priorities effectively.
  • Proficient computer skills, including electronic health records (EHR) and Microsoft Office applications.
  • Advanced Certifications in Hospice and Palliative Nurse certification by the 3rd year of employment.
  • All offers of employment are conditioned upon the result of a criminal background check conducted by the Iowa Division of Criminal Investigation, as required by law and a negative finding on the Office of Inspector General’s Excluded Provider List.

Responsibilities

  • Conduct comprehensive OASIS assessment visits to evaluate patient health status.
  • Determine clinical eligibility for home health services.
  • Develop and implement individualized, patient-centered care plans.
  • Educate patients and families on disease management and treatment.
  • Develop rapport with referral sources, including physicians, hospitals, and discharge planners.
  • Assess patients' health conditions and develop individualized care plans in collaboration with physicians and other healthcare professionals
  • Collaborate with interdisciplinary teams to ensure seamless communication and optimal patient outcomes
  • Ensure compliance with healthcare regulations, safety standards, and agency policies
