About The Position

The Home Health Administrative LVN/LPN plays a critical role in delivering high-quality nursing care and administrative support within a home health care setting. This position ensures that patients receive personalized, compassionate care while maintaining accurate clinical documentation and compliance with healthcare regulations. The role involves coordinating patient care plans, managing communication between healthcare providers, patients, and families, and facilitating smooth operational workflows. Requirements

  • Current and valid Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license in the United States.
  • Minimum of 1 year experience in nursing, preferably in home health care or a related clinical setting.
  • Strong knowledge of nursing principles, patient care standards, and healthcare regulations.
  • Proficiency in electronic health records (EHR) systems and basic computer skills.
  • Ability to communicate effectively with patients, families, and healthcare professionals.

Nice To Haves

  • Experience with home health care administrative duties including scheduling and billing.
  • Certification in Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS).
  • Familiarity with Medicare and Medicaid home health regulations and documentation requirements.
  • Experience working with interdisciplinary healthcare teams in a community or home care setting.
  • Additional training or certification in patient education or chronic disease management.

Responsibilities

  • Conduct comprehensive patient assessments and document clinical findings accurately in patient records.
  • Coordinate and schedule home visits, ensuring timely delivery of nursing care and services.
  • Serve as a liaison between patients, families, physicians, and interdisciplinary care teams to facilitate effective communication and care planning.
  • Maintain compliance with state and federal healthcare regulations, including documentation standards and patient privacy laws.
  • Manage administrative tasks such as billing support, insurance verification, and inventory management of medical supplies.
  • Provide patient education and support to promote adherence to treatment plans and improve health outcomes.
  • Assist in developing and updating care plans based on patient progress and clinical guidelines.
  • Monitor and report any changes in patient condition to the supervising nurse or physician promptly.
