About The Position

The Home and Community Habilitation Specialist - Enhanced - will provide quality care and goal-oriented activities to individuals at home and in the community as outlined in their ISP. This is a diverse position that may vary greatly depending upon the needs of the individual and/or their family.

Requirements

  • At least eighteen (18) years of age
  • Valid driver's license with a three (3) year clean driving record
  • Ability to walk, bend, stoop, sit, stand, reach, and lift items weighing 50 pounds or less
  • Satisfactory completion of pre-offer physical examination, applicable background clearances and post offer as required
  • Ability to read and comprehend the English language to understand work assignments, LifePath policies and procedures, and perform job safely
  • LPN or RN License (if the individual needs nursing services) or if the individual does not need nursing services: must have one of the following certifications or degrees: NADD Competency Based Clinical Certification, NADD Competency Based Dual Diagnosis, NADD Competency Based Direct Support Professional Certification, Registered Behavior Technician, Certified Nursing Assistant or Registered Nurses Aide, Board Certified Assistant Behavior Analyst, Bachelor's Degree or higher in Psychology, Education, Special Education, Counseling, Social Work, Gerontology or Therapeutic Recreation

Responsibilities

  • Provide quality care to individuals in an environment which promotes their rights, dignity, freedom of choice, and their individuality as illustrated by the following:
  • Provide 1:1 attention, which encourages each individual's ability to maintain or attain the highest practical physical, mental, and psychosocial well-being.
  • Knowledgeable of the ISP for each individual and provide support to the individual according to the care plan. Contribute to the care planning process by providing the appropriate personnel with specific information and observations of the individuals' needs and preferences.
  • Attend to the individual needs of the individuals, which may include assistance with personal hygiene, feeding, incontinent care, toileting, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individual's care requirements.
  • Maintain the comfort, privacy, and dignity of each individual in the delivery of services to them. Interact with individuals in a manner that displays warmth and promotes a caring environment.
  • Fully understand all aspects of individuals' rights, including the right to be free of restraints and free from abuse. Responsible for reporting to the Supervisor and/or appropriate personnel any incidents or evidence of individual abuse or violation of individuals' rights.
  • Protect the personal belongings of each individual, including furnishings, jewelry, clothing, memorabilia, etc. Promptly report missing items according to the established policy and participate in efforts to locate missing items.
  • Observe individuals for changes in condition or behavior and promptly report these changes to appropriate personnel.
  • Orient individuals and their families to the program upon admissions and/or transfer.
  • Lift, move, and transport individuals using proper body mechanics or lifting devices for accident prevention.
  • Communicate and interact effectively and tactfully with the individual, visitors, families, peers, supervisor, people in the community and other personnel.
  • Assist with and escort individuals to appointments as needed. Participate in activities as directed.
  • Complete documentation of care provided or other information in keeping with program policies.
  • Perform all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves, and proper disposal of soiled materials.
  • Responsible for utilizing department petty cash wisely and within the parameters of the petty cash policy as well as within the scope of the individuals' goals and ISP.
  • Follow the Department schedule to ensure that all individuals are receiving their weekly allocated units of service. Work shifts as scheduled by supervisor.
  • Develop and maintain an open channel of communication with family/providers regarding the individuals' needs, goals, and progress.
  • Submit assigned paperwork daily or as requested by Department Supervisor and/or by its assigned deadline.
  • Develop and schedule meaningful outings and activities for individuals that promote progress towards their goals as written in their ISPs.
  • Report any safety issues to the Department Supervisor immediately
