About Your Role: As a Qualified Mental Health Professional, you will offer direct supportive services to adults, children/youth and families with mental, behavioral, or emotional disturbances resulting in significant functional impairment. Providing education, training, and support to your clients in the home and community, you will engage them in change, help sustain their progress and enable them to become more successful in their family unit. Provide direct services such as escorting clients to appointments/activities, parenting support, life skills training, therapeutic mentoring, and behavior plan implementation. Actively engage clients and families in carrying out the Individualized Service Plan (ISP) as directed by the supervisor. Build professional, supportive relationships while maintaining appropriate boundaries. Document all services accurately and within required timeframes in the Electronic Health Record (EHR). Maintain effective collaboration with external professionals (schools, courts, providers, etc.). Participate in treatment planning meetings and case coordination activities. Attend and contribute to weekly supervision, staff meetings, and required trainings. Submit all required documentation (e.g., timesheets, mileage forms, client notes) by the stated deadlines each pay period. Serve as a positive role model for clients and families.
