About The Position

The PACE Home Care Tech provides direct care to participants in their homes, contracted facilities, or at the PACE Center under the supervision of the RN-Home Care Coordinator. This role involves assisting with personal care, collecting and documenting participant data, and supporting therapists with skilled therapy and restorative maintenance care.

Requirements

• High School Diploma or equivalent.
• 1 year of Nurse tech hospital experience or skilled nursing facility experience, specifically with the frail or elderly.
• CPR certification.
• Valid driver's license with an acceptable three-year motor vehicle record and a certificate of auto insurance.

Nice To Haves

• Basic computer skills.
• Knowledge of office equipment such as fax and copier.

Responsibilities

• Provides personal care including bathing, dressing, feeding, toileting, transfers, lifting, and incontinent care to PACE participants in their homes.
• Provides care to participants at the PACE Center.
• Reports any changes in medical or physical status to the Home Care Supervisor PACE or designee.
• Performs procedures/treatments under the supervision of HCS or RN.
