Requirements

  • Willingness to provide approximately 12 hours per week of direct client care
  • Strong organizational and problem-solving skills
  • Ability to manage multiple staffing priorities in a fast-paced environment
  • Excellent communication and relationship-building skills
  • Active STNA, CPR certification, and current TB test
  • Valid driver’s license with reliable, insured transportation

Nice To Haves

  • Social Worker or Nursing Degree is a PLUS
  • Home care scheduling or caregiver staffing
  • Experience with AxisCare or similar home care software
  • Background in home care, hospice, assisted living, or healthcare operations
  • Experience as a Social Worker, Case Manager, Care Coordinator, Discharge Planner, or Patient Advocate

Responsibilities

  • Caregiver Scheduling & Staffing Coordination: Manage caregiver schedules using AxisCare Software. Ensure full client coverage and prevent missed shifts. Respond to call-offs and last-minute staffing needs. Maintain a reliable backup caregiver pool. Monitor overtime and scheduling efficiency.
  • Client Care Coordination: Match caregivers to clients based on skills, personality, and care needs. Understand client care plans and home environments. Support client satisfaction and caregiver retention. Serve as a liaison between clients, families, caregivers, and healthcare providers.
  • Caregiver Support & Field Operations: Address caregiver and client concerns. Resolve scheduling or care conflicts. Support caregivers in the field. Protect client relationships and service continuity.
  • Staffing & Caregiver Development: Identify caregiver training needs. Coordinate training with the Hospice Support Nurse. Identify staffing gaps and hiring needs.
  • Hospice Coordination: Ensure properly trained caregivers are assigned to hospice clients. Coordinate with hospice partners when appropriate.
  • Field Engagement (Client Care): Provide approximately 12 hours per week of direct client care. Support clients and caregivers in the field. Assist with coverage when needed.
  • Compliance & Documentation: Ensure caregiver documentation standards are met. Confirm documentation is uploaded in AxisCare. Maintain caregiver file completeness. Support compliance with Ohio non-medical home care regulations
