About The Position

The Home Care Scheduler at BayCare HomeCare is responsible for scheduling and overseeing various disciplines including nursing, therapy, and social work. This role is crucial in ensuring that patient care is coordinated effectively and that all necessary information is accurately entered into the scheduling and billing systems. The position requires strong organizational skills and a commitment to providing outstanding customer service to the community.

Requirements

• High School diploma or equivalent required.
• 1 year of computer-related experience required.

Nice To Haves

• Associate's degree in a computer-related field preferred.
• 1 year of home care experience preferred.

Responsibilities

• Responsible for scheduling and oversight for all disciplines: Nursing, Therapy, and Social Work.
• Assists in entering approved PTO and patient Emergency Management Plans into scheduling package.
• Enters and verifies vital patient information into billing system.
• Maintains CPR card, Driver's license and Auto Insurance information in team member files.
• Reviews admission packets for completeness.
• Assists the Home Care Supervisor in productivity oversight.
• Works all report as indicated in standard work.
• Adds and approves all disciplines as instructed by the Supervisor, including the visit type.
• Enters skills for each clinician in staff management area of scheduling package and enters assignment of and updates for case manager for each patient.
• Assists with daily routine paperwork and clerical duties necessary for the smooth day to day function of the unit.
