About The Position

The Home Care Scheduler is responsible for coordinating and scheduling Home Health and Hospice services for clients, ensuring continuity of care by matching clinicians with the appropriate competencies to meet client needs. This role requires effective communication with various stakeholders, including patients, clinicians, and clinical managers, and involves exercising judgment in scheduling decisions. The position is remote and requires local applicants from the Philadelphia area, with a commitment of 40 hours per week and one weekend per month.

Requirements

• Computer efficiency with working knowledge of Microsoft Office Suite.
• Home Care Scheduling experience preferred.
• Exceptional verbal and written communication skills.
• Experience in a healthcare-related field or home care experience preferred.
• Attention to detail.

Responsibilities

• Schedule clients for Home Health and Hospice services ensuring clinician continuity.
• Identify client needs and match them with clinicians who have the appropriate competencies.
• Exercise judgment in determining effective scheduling and rescheduling methods.
• Maintain optimal communication with patients, clinicians, clinical managers, physicians, and homecare coordinators.
• Provide coverage for multiple sites and/or Hospice support as needed.
