About The Position

The Home Care Nurse is responsible for the planning, coordination, provision, supervision, and evaluation of clinical services for Osborn Home Care. This role requires a professional demeanor and a positive attitude while supporting the organization's mission and policies.

Requirements

• Excellent customer service and communication skills with all parties involved in client care.
• Organized and able to prioritize, track, and schedule visits with necessary follow-ups and follow-through.
• Proactive in ensuring informed and effective client care teams.
• Proficient in documenting.
• Adept in problem solving.
• Technologically capable.
• Must have NYS or CT RN.

Responsibilities

• Visit and assess clients and their environments.
• Implement care plans to ensure quality of care.
• Supervise, coach, and educate Home Health Aides.
• Coordinate care with scheduling coordinators, physicians, families, and other interdisciplinary team members.
• Obtain physician orders for all services, medications, and treatments.
• Manage medications and treatments when applicable.
• Perform ongoing assessments of clients and their home health aide needs.
• Document per Department of Health guidelines.
