About The Position

As an RN Case Manager at Adara Home Health, you will be responsible for managing client care schedules and providing direct nursing care to clients in their homes. This full-time position involves performing assessments, developing care plans, and collaborating with healthcare professionals to ensure positive outcomes for clients. You will also serve as an advocate for clients, ensuring their safety and well-being while managing documentation and compliance with healthcare standards.

Requirements

  • Currently licensed as a Registered Nurse (RN) in the State of Minnesota without limitations or restrictions.
  • Proof of negative TB testing (mantoux skin test, single TB blood test, or chest x-ray).
  • Access to a dependable automobile for travel to multiple business stops.
  • Valid driver's license and proof of current car insurance when using a personal vehicle for company business.
  • U.S. Citizenship or valid Alien Work Permit.
  • Disclosure of any conviction and criminal history records related to health services provision.
  • Pass initial and ongoing background studies and screenings.
  • At least two (2) years' experience in a healthcare facility or home care field.
  • Demonstrates critical thinking skills and the ability to follow standards/procedures for safe client care.
  • Ability to work independently with minimal supervision and manage a variety of assignments.
  • Excellent written and oral communication skills; interpersonal communication and teaching skills.
  • Basic computer skills and willingness to learn new software programs; experience with Electronic Medical Records and Microsoft Outlook preferred.
  • Current CPR certification highly recommended.

Responsibilities

  • Manage client schedules Monday-Friday with an on-call rotation every 5th weekend and 1 week night every other week.
  • Perform initial and periodic assessments of clients to determine care needs and evaluate the effectiveness of care.
  • Collaborate with physicians and the home health team to develop and update the client's home health plan of care at least every 60 days.
  • Serve as a client advocate by assessing the home environment and developing individualized abuse prevention plans.
  • Meet documentation standards ensuring accuracy, timeliness, and compliance with payer/billing requirements.
  • Provide education to clients, families, caregivers, and unlicensed personnel as needed.
  • Provide direct care to clients of varying ages and clinical acuity levels in accordance with company policies and the Minnesota Nurse Practice Act.
  • Coordinate client care with family, caregivers, community resources, and other healthcare members.
  • Provide clinical oversight, delegation, and leadership to licensed clinicians and unlicensed personnel.
  • Ensure effective financial management of the client's home health services.
  • Manage work schedule effectively and adapt to unexpected changes.
