About The Position

What You’ll Do Provide direct patient care, including assessments and care plan development Manage a caseload and coordinate care across the clinical team Administer medications and treatments per physician orders Monitor patient progress and adjust care plans as needed Educate patients and caregivers on care and self-management Communicate with physicians and care teams to ensure continuity of care Complete accurate and timely documentation (OASIS, visit notes, orders) Ensure compliance with policies, regulations, and HIPAA standards Join a team where you can deliver meaningful, patient-centered care right where it matters most.

Requirements

• Active RN license
• Graduate of an accredited nursing program (BSN preferred)
• Home health or related behavioral health clinical experience
• Strong communication, organization, and critical thinking skills
• Current CPR certification
• Valid driver’s license and reliable transportation
