As an RN Case Manager, you will play a crucial role in delivering direct care while overseeing and directing home health services for clients within the community. This position requires collaboration with clients, their families, and the healthcare team to develop and implement goal-centered care plans that consider both quality measures and the financial aspects of treatment. You will provide skilled nursing care to clients and offer clinical oversight to both professional and unlicensed personnel, ensuring that all care provided meets the highest standards of quality and safety. In this role, you will be responsible for performing initial and periodic assessments of clients to determine their care needs, implementing necessary interventions, and evaluating the effectiveness of care to achieve positive outcomes. You will collaborate with physicians and the home health team to develop and update the clients' home health plans of care at least every 60 days or more frequently as changes occur. As a client advocate, you will ensure that safety needs are addressed by assessing the home environment, developing individualized abuse prevention plans, and reporting any suspected vulnerabilities such as abuse, neglect, or financial exploitation. You will also be responsible for meeting documentation standards, ensuring accuracy, timeliness, and compliance with payer and billing requirements. Education will be a key component of your role, as you will provide guidance to clients, families, caregivers, and both licensed and unlicensed personnel. The RN Case Manager will coordinate client care with family members, caregivers, community resources, and other healthcare professionals, providing clinical oversight, delegation, and leadership to ensure effective and appropriate financial management of the clients' home health services. Flexibility is essential, as you will manage your work schedule effectively and participate in an on-call rotation as needed.
