About The Position

Rocky Mountain Care is seeking a dedicated Registered Nurse (RN) Case Manager to provide compassionate health care services in the Utah County area. This full-time position allows for flexible scheduling, enabling nurses to manage their visits according to their availability. The RN Case Manager will be responsible for assessing resident needs, developing individual care plans, administering nursing care, and supervising Certified Nursing Assistants (CNAs).

Requirements

• Current, unencumbered RN license in Utah.
• BLS Certification.
• 1 year of nursing experience.
• Mid-level analysis skills.
• Case management experience preferred.
• Strong communication skills, both written and oral.
• Basic computer skills.

Nice To Haves

• Experience in nursing administration.
• Positive attitude and desire to work with the aging population.

Responsibilities

• Deliver nursing care to a group of residents in a nursing unit.
• Assess resident needs and develop individual care plans.
• Administer nursing care and supervise CNAs and other personnel in nursing care delivery.
• Make independent decisions when circumstances warrant such action.
• Collaborate with an interdisciplinary care team to ensure effective care delivery.
