About The Position

St. Mary Home Care, a member of Trinity Health at Home, is seeking a Full-time Registered Nurse Case Manager for the greater Bucks County/Upper Bucks County area. This role involves delivering skilled nursing care in the comfort of patients’ homes, creating individualized care plans, administering medications, educating patients and families, and supporting recovery and independence. Whether experienced in home health or transitioning from hospital, acute care, or skilled nursing, this position offers the chance to practice at the top of your license with greater flexibility, autonomy, and meaningful 1:1 patient relationships. Requirements

  • Graduate of an approved nursing education program
  • Licensure as a Registered Nurse in the state of PA
  • One (1) year experience as a professional care nurse
  • Must have current Driver’s license and reliable transportation
