About The Position
We are seeking a compassionate and reliable Home Care Nurse (RN/LPN) to join our team. In this role, you will provide skilled nursing care to patients in their homes, ensuring comfort, safety, and improved health outcomes. You will work closely with physicians, therapists, and other healthcare professionals to deliver individualized care plans.
Requirements
- Current, active RN or LPN license in the state of practice

Responsibilities
- Provide skilled nursing care in accordance with physician orders and care plans
- Perform patient assessments and ongoing evaluations
- Administer medications, treatments, and wound care as prescribed
- Monitor and document patient progress accurately and in a timely
- Educate patients and families on disease management and care techniques
- Coordinate care with interdisciplinary team members
- Ensure compliance with all state, federal, and agency regulations
