About The Position

Metro Physical & Aquatic Therapy is a comprehensive multidisciplinary company offering Physical, Occupational, Speech Therapy, Massage, and Acupuncture services. They are seeking a motivated and passionate PER DIEM Physical Therapist to provide in-home care. Requirements

  • Physical Therapist Program Graduate
  • Licensed Physical Therapist in the state of New York or eligible
  • Mandatory Certifications: First Aid, AED, & CPR Certifications
  • Ability to travel to patients’ homes within the assigned geographic territory

Nice To Haves

  • Home health experience is preferred but not required

Responsibilities

  • Accept and treat patients within your designated geographic area
  • Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
  • Develop a plan of care and treatment goals in consultation with the physician, patient, and family
  • Plan, prepare and implement customized therapy programs to restore function and promote independence
  • Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
  • Provide timely and accurate documentation for each visit
  • Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
  • Deliver care in compliance with agency policies and professional practice standards
