The Home Care Physical Therapist is a professional who coordinates and directs the home care patient's care in their home environment (private residence, assisted living, etc.) based on individual needs. The Home Care Physical Therapist is responsible for independent management of the Home Care patient population requiring use of advanced assessment, teaching, and decision-making skills. The Home Care Physical Therapist cares for a caseload of home care patients by evaluating the patient for appropriateness of home care needs and developing the plan of care in collaboration with the provider, caregivers, and Home Care IDG members. The Home Care Physical Therapist provides follow up by evaluating the effectiveness of the plan of care and monitoring the patient and caregivers response to the plan. The Home Care Physical Therapist provides patient care based on provider's orders, departmental policies and procedures, regulatory guidelines, and best practice. The Home Care Physical Therapist documents in the patient's medical record in a timely manner according to agency policy. The Home Care Physical Therapist attends and actively participates in Case Conference and team meetings. The Home Care Physical Therapist performs on-call responsibilities. The Home Care Physical Therapist supervises and delegates appropriate functions to the Physical Therapist Assistants.
