Metro is looking for a motivated and passionate PER DIEM Physical Therapist Assistant to provide in-home care under the supervision and direction of a licensed Physical Therapist. Requirements

  • Physical Therapist Assistant Program Graduate
  • Licensed Physical Therapist Assistant in the state of New York or eligible.
  • Mandatory Certifications: First Aid, AED, & CPR Certifications.
  • Ability to travel to patients' homes within the assigned geographic territory

Nice To Haves

  • Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.

Responsibilities

  • Accept and treat patients within your designated geographic area
  • Provide physical therapy treatments to patients in their homes, as outlined in the licensed PT's plan of care
  • Monitor and report changes in patient status to the supervising PT in a timely manner
  • Communicate effectively with patients, caregivers, and interdisciplinary team members to ensure coordinated, effective care
  • Instruct patients, family members, caregivers, and Home Health Aides in exercises, safety awareness, and use of assistive devices as directed by the supervising PT
  • Assist the PT in updating care plans based on patient progress and response to treatment
  • Document patient treatments and observations accurately and promptly, in accordance with agency and regulatory standards
  • Participate in case conferences and contribute to the care planning and discharge process, in collaboration with the PT
  • Adhere to all federal, state, and local regulations as well as agency policies and professional standards of practice
