About The Position

Evaluate and treat patients needing OT services in the home in the Waterbury (Team 5) Region. Attend regular team meetings and coordinate care with other disciplines. Position Purpose: Provides professional occupational therapy services to the agency’s clients in accordance with the plan of care as prescribed by the physician and in compliance with the state’s professional practice act for occupational therapy and any applicable licensure/certification requirements. Position Details: Schedule: Monday - Friday, 8 am - 4:30 pm, no nights, weekends or on call requirements Coverage Region: Waterbury region NOTE: This position is 100% home care providing services in patients homes.

Requirements

  • Graduate of an occupational therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association and the American Occupational Therapy Association.
  • Eligible for the National Registration Examination of the National Board for Certification in Occupational Therapy (NBCOT).
  • Registration by the NBCOT and registration to practice as an Occupational Therapist in state of Connecticut.
  • Must be able to function in an environment with minimal direct supervision, accepting personal responsibility for maintaining professional relationships with the agency’s clients and their families
  • Must accept responsibility for maintaining clinical practice skills, learning and adhering to agency’s policies and procedures on an on-going basis.
  • Must have a minimum of one (1) year experience.
  • Must have current Driver’s license and reliable transportation to and from work site.

Nice To Haves

  • Home healthcare experience preferred.

Responsibilities

  • Effectively and efficiently prioritizes visit content and plans visits based on client
  • Maintains a working knowledge of applicable Federal, State and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures and adheres in a manner that reflects honest, ethical and professional behavior.
  • Accurately assesses clients at admission, at required time points, and as needed, utilizing appropriate standardized documentation.
  • Assesses the client’s condition and needs and determines the agency’s ability to meet those needs in the home care setting.
  • Develops appropriate assessment-based plan of treatment with physician and client.
  • Plan includes attainable episode goals and agency services needed to improve and/or stabilize client’s medical condition and promote independence.
  • Participates in timely revision of the plan of treatment as appropriate.
  • Makes appropriate referrals for evaluation/care to other disciplines and services, and coordinates care with others to ensure effective and efficient care is provided.
  • Utilizes interview, observation and evaluation in assessing clients and applies professional judgment, consistent with practice standards, in formulating interventions and making recommendations to the physician and client.
  • Evaluates the home environment and makes recommendations for the establishment of effective household management.
  • Initiates and implements appropriate preventive, therapeutic and rehabilitative procedures and techniques in accordance with the plan of treatment.
  • Assists and instructs client and family members in activities and concepts to promote the client’s health, safety, and independent function.
