Eddy Visiting Nurse & Rehab Association has an exciting opportunity for a Full-Time Occupational Therapist responsible to manage a caseload of patients requiring home-based therapy- primarily post acute or subacute- in their homes. **Experience the rewards of appreciation from being able to address the whole patient**

Requirements

• NYS Occupational Therapist Licensure
• 1 year Occupational Therapy experience is required
• Valid Driver's License and Reliable Vehicle

Nice To Haves

• Home care experience preferred but not required

Responsibilities

• Promote/provide Occupational Therapy services in the home to assist the patient in achieving his/her optimal level of functioning.
• Evaluate and treat individuals with functional deficits secondary to neurologic, orthopedic and medical conditions.
• Teach patients/caregivers and help them to become independent with their treatments and self-management of their chronic illness(es).
• Coordinate an interdisciplinary plan of care with nursing/physical/occupational/speech therapists, medical social workers, aides and dieticians.
