About The Position

The PRN Home Care Nurse is a practice-based Registered Nurse (RN) who directly supports Inspiritus’ highest risk clients who are authorized to receive nursing services. In collaboration with other members of the clinical service team, the Home Care Nurse is responsible for completing in-person comprehensive assessments for individuals with Intellectual and Developmental Disabilities (I/DD). The ideal candidate will be a team player open to collaboration with individuals receiving services, Host Home Providers, Nursing Services Coordinator, and Disability Services team members. This is a PRN, hourly position that requires prior direction and approval from the Nursing Services Coordinator, Assistant Director, or Executive Director before proceeding with an assignment. Responsibilities include, but are not limited to: conducting comprehensive assessments, completing documentation, conducting education and training, and clear communication. The PRN Home Care Nurse will work closely with and report to the Nursing Services Coordinator. This position requires travel to service participants’ homes and/or day programs. Requirements

  • Valid and current RN license or greater clinical license
  • Valid and current Georgia driver’s license in good standing; current auto insurance
  • 2 years’ experience; 3-5 years working with individuals with I/DD preferred
  • Comprehensive nursing assessment experience, problem identification and care plan development
  • Screening for developmental issues, depression, and other psychological conditions
  • Ability to communicate with all persons in an effective, respectful, culturally competent, and professional manner
  • Current or able to obtain BLS/First Aid Certification
  • Current or able to obtain CPI Certification

Nice To Haves

  • Knowledge of the Department of Behavioral Health and Developmental Disabilities (DBHDD) rules and regulations - preferred
  • Knowledge of Person-Centered Planning and Thinking - preferred

Responsibilities

  • Conduct comprehensive in-person assessment of clients’ physical, mental, and psychosocial needs
  • Provides person-centered education to individuals and Host Home Providers as required
  • Provide in-person training to Host Home Providers as required
  • Document in Electronic Health Record system, if directed
  • Travel to clients’ homes and/or day programs to conduct assessments
