About The Position

Works in cooperation with the interdisciplinary team to effectively implement the Plan of Care. Provides nursing care in accordance to medical orders and the Plan of Care. Nursing care is provided for patients enrolled in the Hospice and Palliative Care of the Piedmont programs. This position is supervised by the Clinical Director. Qualifications: Associate Degree or Diploma in Nursing. Bachelors Degree preferred A minimum of two years experience in a medical surgical environment, previous home health or hospice experience preferred Ability to assess and respond to the complex needs of terminally ill patients and their family Skilled in nursing practice, able to provide emotional support and acceptable of individual lifestyles Skilled in pain and symptom management Professional, positive attitude required Ability to function as part of a team Ability to embrace the mission and culture of the organization

Requirements

  • Associate Degree or Diploma in Nursing

Nice To Haves

  • Bachelors Degree preferred
  • previous home health or hospice experience preferred

Responsibilities

  • Provides care of assigned patients by making visits to patients within the service area and by providing nursing care which includes patient/family teaching and support
  • Communicates regarding the patient's status with physician, RN Case Manager, team members and other agencies involved in the care of the patient
  • Attends team conferences, staff meetings and in-services as assigned
  • Documents patient/family contacts, nursing care provided and other required documentation in an accurate and timely manner
  • Adheres to all regulatory standards and agency policies as set forth
  • Participates as assigned in the 24 hour on-call as scheduled
  • Assists with referrals and evaluation visits, as needed.
  • Supports the overall culture of the organization
  • Participates in organization sponsored events
  • Performs other related duties as assigned
