About The Position

Responsibilities As the Home Care Social Worker, your primary responsibility is to contribute meaningfully to the treatment of the patient’s medical conditions or rate of recovery. To resolve social or emotional problems which are or expected to be an impediment to the patient’s medical condition. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Current state license in Social Work. 2. Experience in working with a diverse population with a wide range of problems (i.e., death and dying, disability, chronic illness, etc.). 3. BLS certificate upon hire and maintain current preferred. 4. Current driver’s license, car insurance, car registration, and reliable automobile.
