About The Position
We are seeking a Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) with experience in home care. The successful candidate will be responsible for medication management, conducting patient assessments, and supervising home care aides to ensure the highest quality of care. Requirements
- Current and valid RN or LPN license.
- Demonstrated experience in home care nursing.
- Strong clinical assessment and medication management skills.
- Excellent communication and leadership abilities.
- Ability to work independently and as part of a multidisciplinary team.
Responsibilities
- Manage and administer medications in accordance with care plans and physician orders.
- Perform thorough patient assessments to monitor health status and identify any changes in condition.
- Supervise and provide guidance to home care aides to maintain standard care procedures.
- Collaborate with healthcare professionals and family members to coordinate comprehensive care.
- Document all care activities accurately and timely.
