About The Position
United Health Services (UHS) is seeking a Licensed Practical Nurse (LPN) to join our team and provide high-quality care to patients in a supportive and collaborative environment. As an LPN at UHS, you will work under the supervision of a Registered Nurse or physician to deliver compassionate, hands-on care, administer medications, monitor patient health, and assist with daily living activities. Requirements
- Minimum Required:
- Graduated an accredited school of Practical Nursing
- Current New York State Practical Nurse License.
- Basic Skills Certifications (CPR) required upon hire.
Responsibilities
- Performs clinical assignments within scope of practice to maximize patient experience and ensure patient safety.
- Promotes and facilitates the compliance and understanding NYS DOH, Infection Control and all regulatory standards.
- Under the direction of a RN, educates patient and family in health promotion and disease management using available resources.
- Utilizes a working knowledge of pharmacology and safely administers medications according to UHS Home Care policies.
- Maintains CLIA Waived Lab standards and performs CLIA Waived Lab testing and proficiency validations according to regulatory standards.
- Collaborates with patients, families, and other members of the healthcare team to coordinate patient care.
- Participates in Performance Improvement activities, reviews PI data and assists in the development of Performance Improvement plans.
- Participates in educational opportunities
- Will perform other duties as assigned.
