About The Position

The Home Care Educator partners with Home Care and Hospice leadership to design, deliver, and evaluate clinical education for professional and nonprofessional staff. This role supports clinical excellence through orientation, competency validation, documentation education, and ongoing development, serving as a role model for ethical, compassionate, patient-centered care. Requirements

  • Bachelor’s degree in Nursing (required)
  • Current RN license in the state of practice
  • 2+ years of home healthcare experience
  • 3+ years of nursing experience total
  • Valid driver’s license and reliable transportation
  • Proficiency with computer office software
  • Strong verbal and written communication skills
  • Commitment to Trinity Health’s Mission and Code of Ethics

Nice To Haves

  • CPR Instructor Certification preferred

Responsibilities

  • Facilitate clinical orientation for new associates, including systems use, laptop documentation, competency validation, and mandatory education
  • Design, implement, deliver, and evaluate education programs using adult learning principles to meet departmental and regulatory needs
  • Provide observation, coaching, and education to clinical staff at the request of leadership
  • Identify, troubleshoot, and resolve clinical documentation issues; ensure timely follow-up and education
  • Support Horizon computer software users and coordinate issue resolution
  • Communicate regulatory updates, policy changes, and best practices to clinical teams
  • Maintain accurate documentation of education plans, competency validation, and staff development in associate files
  • Assist leadership in identifying learning opportunities and developing individualized clinical development plans
  • Make home visits as needed to provide direct patient care and/or nursing supervision
  • Serve on committees, councils, and participate in community outreach representing Trinity Health At Home
  • Stay current in area of clinical expertise and ensure agency practices align with evidence-based standards
  • Support professional growth by sharing knowledge through in-services, presentations, and precepting staff
