The Director of Home Care / Consumer Directed Personal Care Services provides strategic and operational leadership for home care service lines including Consumer Directed Personal Care (CDPC), Medicaid waiver and HCBS services, and private duty home care across multiple locations. Requirements

Leadership experience in home care including consumer-directed, waiver, or private duty services
Knowledge of Medicaid waiver and HCBS regulations
3–5+ years leadership experience
Strong operational and communication skills
Clinical license not required

Nice To Haves

Multi-site leadership experience preferred

Responsibilities

Provide multi-site leadership of consumer-directed, waiver-based, and private duty home care programs
Lead Consumer Directed Personal Care models ensuring participant choice and compliance
Oversee Medicaid waiver and HCBS program operations
Direct private duty home care staffing, scheduling, and service delivery
Support start-ups and market expansion initiatives
Implement standardized operational policies and workflows
Oversee caregiver recruitment, onboarding, and retention
Track KPIs related to utilization, compliance, quality, and satisfaction
Partner with finance, compliance, and operational leadership
Develop future operational leaders
