Home Care Coordinator LPN - Full Time Under the direct supervision of the Team Lead or Center Manager and the Home Care Agency Director RN, the EveryAge PACE Home Care Coordinator LPN is responsible for the development and implementation of homecare services for program participants, including the coordination of all Durable Medical Equipment and orchestrates the delivery of health care services for PACE participants in all care settings according to their plan of care. Directs and supervises the Home Health Aide and the Personal Care Attendants (PCAs) in the provision of activities of daily living, day center activities, restorative and supportive care of the EveryAge PACE participants. When appropriate, provides health education for participants in their home. Communicates participant’s changes and progress to appropriate staff.

Requirements

Graduate of a school of professional nursing.
Current North Carolina Licensed Practical Nurse (LPN) License,
Cardiopulmonary Resuscitation (CPR) certification required.
Minimum of one year of experience providing skilled nursing care.
One year of working with the elderly

Responsibilities

Assists with the scheduling of initial and periodic assessments in participants’ living environment and at the PACE Center (minimally every six months) to facilitate in developing and monitoring of each participant's plan of care.
Acts as home care liaison to the IDT, gives report of the medical, functional, and psychosocial condition of participants as needed.
Coordinates nursing care in the home through collaboration with other health care professionals by ensuring services delivered are consistent with plan of care.
Identifies and addresses medication plan adherence throughout all care settings.
Coordinates transitions of care by maintaining cooperative working relationships with outside facilities (hospitals, rehabilitation facilities, and skilled nursing facilities), caregivers, and team members.
Coordinates home care coverage based on participants’ plan of care.
Coordinates 24-hour care delivery and the implementation of all home care services which include personal care services to ensure that quality services are provided to meet participant needs.
Coordinates and authorizes all Durable Medical Equipment. Manages home supplies such as incontinence, diabetic, colostomy, nutritional, and other supplies.
Assesses home for pest infestations and coordinates with exterminator to schedule and treat when necessary.
Reconciles invoices for home supplies.
Coordinates any DME for participants in nursing facilities according to program standards. Includes interaction with other facility staff. Assists with discharge planning efforts from nursing facility to home and arranges appropriate care as part of discharge plan as approved by IDT.
Participates, collaborates and contributes as a member of the Interdisciplinary team, emphasizing teamwork and collaboration in all clinic and Interdisciplinary team interactions.
Communicates and works efficiently with the IDT when evaluating potential enrollees.
Participates in participant care planning including the implementation of SMART goals/interventions for the participants’ care plans and enters all care plan information in a timely manner as per organizational protocols. Updates participants’ care plans appropriately throughout the reassessment period. Attends staff meetings.
Works closely with the clinic RN and PCP regarding in-home nursing needs.
Works with Social Worker to provide community resources for participant needs.
Educates and supports participants and families toward recognizing and resolving physical, emotional, and environmental health problems.
Maintains accurate and timely documentation of all services provided through competent use of the electronic medical record and according to EveryAge PACE policy and procedures.
Participates in quality management program activities as requested.
Maintains safe environment utilizing universal precaution and complying with hazardous waste policies and procedures.
Keeps confidentiality of participant records, reports, and discussions in accordance with HIPAA law.
Performs other duties as assigned.
