About The Position

The Home Care Clinical Nurse is responsible for providing nursing care and services to home care patients in accordance with the CHOP, CHHC standards of practice, policy and procedure and within all regulatory and accrediting agency guidelines. Utilizes the nursing process to deliver nursing care to patients and families.

Requirements

Technical Diploma Nursing Required
Pediatric or Adult acute, subacute, other inpatient or Home Care experience Required
Ability to operate a company vehicle and/or be insured on a CHOP policy. (Required proficiency)
Strong interpersonal and customer service skills (Required proficiency)
Strong verbal and communication skills (Required proficiency)
Strong clinical judgment skills (Required proficiency)
Ability to work independently (Required proficiency)
Ability to work as a team player in a fast paced environment (Required proficiency)
Data entry skills (Required proficiency)
Driver's License - Department of Motor Vehicles - upon hire - Required
Registered Nurse (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required
Registered Nurse (New Jersey) - New Jersey State Licensing Board - within 9 months - Required

Nice To Haves

Bachelor's Degree Nursing Preferred
At least two (2) years of inpatient nursing with pediatrics/adults Preferred
At least one (1) year of home care experience Preferred

Responsibilities

Provides competent care for patients and families based on experience.
Nursing practice and documentation is in accordance with CHOP, CHHC standards of care, policy &procedure as well as all regulatory and accrediting agency guidelines.
Participates in on -call rotation to meet patient care needs.
Demonstrates competence in professional practice.
Demonstrates an awareness of research and its value to clinical practice.
Participates in efforts to apply research findings to practice.
Participates in the development of professional and educational activities.
Examines successes and failures to improve performance.
Maintains a blame free environment.

Performs other duties as assigned by Home Care Management.
