About The Position

Eminence Home Care is seeking compassionate and dedicated Home Care Caregivers/Direct Care Workers with driving capabilities to join our team. In this specialized role, you will provide exceptional in-home care while also assisting clients with transportation needs, ensuring they attend medical appointments, social outings, and other essential engagements safely and comfortably. Your role is critical in promoting independence and improving the quality of life for our clients by delivering personalized daily living assistance, companionship, and emotional support. You will collaborate closely with clients, their families, and healthcare professionals to implement tailored care plans with professionalism, empathy, and respect. Requirements

• High school diploma or equivalent; additional caregiving certifications are a plus.
• Valid driver's license with a clean driving record; reliable vehicle required.
• Previous experience in home care or related caregiving roles is preferred.
• Excellent interpersonal and communication skills.
• Physically able to assist clients with transfers and mobility.
• Demonstrated compassion, patience, and professionalism.
• Flexible availability, including evenings and weekends as needed.

Responsibilities

• Assist clients with daily living activities such as bathing, grooming, and meal preparation.
• Provide companionship and emotional support to enhance clients' well-being.
• Monitor and document clients’ health status, reporting any changes promptly.
• Administer medications as directed and comply with established care plans.
• Drive clients safely to medical appointments, errands, and social activities.
• Maintain client transportation logs and ensure vehicle cleanliness and safety.
• Communicate effectively with clients, family members, and healthcare professionals about care needs and progress.
• Perform light housekeeping to maintain a safe and clean environment.
