About The Position

Humboldt Senior Resource Center is looking for a Home Care Assistant to support the in-home care services in the Fortuna area, under Redwood Coast PACE. This position is full time with a schedule of 8:30 am to 5:00 pm, Monday through Friday. JOB SUMMARY: Under the supervision of the Home Care Manager, the Home Care Assistant provides general and specialized administrative support to the Home Care department and orders home care supplies, and durable medical equipment (DME).

Requirements

  • High school diploma or GED equivalent required.
  • At least two (2) years of customer service and/or office experience.
  • General administrative and professional support skills.
  • Excellent customer service skills.
  • Works effectively with staff in the department to which assigned, other departments, and others such as participants, family members and contracted providers.
  • Courteous and professional communication.
  • Highly organized.
  • Effectively utilizes Microsoft Office and other relevant computer programs.
  • Works independently and with minimal supervision.
  • Maintains excellent attendance and punctuality records.
  • Work in a constantly changing environment with individuals of various backgrounds and abilities.
  • Physical abilities sufficient to move between different staff work areas, communicate with co-workers and the public, operate a computer for long periods of time, produce reports, talk on the telephone, and potentially travel to other agency worksites.
  • Requires clearance of a DOJ and FBI criminal history background check
  • Pre-employment physical and PPD skin test.

Nice To Haves

  • At least one (1) year of experience working with a frail or elderly population preferred.
  • Valid California Driver’s License, preferred.
  • COVID-19 and Influenza (flu) vaccines preferred.

Responsibilities

  • Answers, screens, and transfers phone calls and emails.
  • Prepares written correspondence, including authorization letters.
  • Performs general clerical duties, including data entry, faxing, and mail distribution.
  • Creates and maintains electronic filing system.
  • Schedules and coordinates meetings for administrative supervisors.
  • Orders supplies (incontinence supplies for participants and PPE equipment for in-home staff) and maintains office supply inventories.
  • Reports any significant participant changes in condition to the Home Care Manager.
  • Follows agency policies for incident reports, grievances, and appeals.
  • Communicates effectively with participants, family members, caregivers, staff, and visitors.
  • Completes documentation accurately and in a timely manner.
  • Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions per the policies and procedures. Takes necessary measures to ensure a safety environment for oneself, co-workers, contractors, participants, visitors, and others.
  • Monitors personal safety and attends to good body mechanics. Ensures infection and exposure control and is prepared in case of emergency.
  • Continually seeks better ways for delivering services and communicating with participants.
  • Demonstrates respect for and promotes participant rights, including dignity, self-determination, access to care, confidentiality, and independence.
  • Understands the importance of community involvement and participates as appropriate in activities that link Redwood Coast PACE to its communities.
  • Effectively collaborates with staff peers and contractors to meet Redwood Coast PACE goals and further success.
  • Complies with all policy and procedures.
  • Demonstrates proficiency in delivering age-specific care.
  • Consistently meets or exceeds quality assessment and performance improvement targets.
  • Participates in the process of teaching, training, and mentoring student interns, and volunteers working within the program area.
  • Participates in on-call coverage in conjunction with the Home Care team.
