About The Position

What is Carolina SeniorCare? Carolina SeniorCare is a P.A.C.E. (Program of All-inclusive Care for the Elderly). A career at Carolina SeniorCare is a calling to help others in their community and help elderly participants (55 or older) stay and live in their respective communities. P.A.C.E. offers a wide variety of services to fulfill this mission such as medical services to medications, from nutritious meals to transportation – all personalized to meet the participant’s specific needs! The Home Care Assistant-Universal Aide is accountable for the delivery of quality home care services to the program's patients in conjunction with physician's directions, company and industry standards, and insurance guidelines as assigned by a registered nurse. The Home Care Assistant is supervised by a licensed nurse to deliver quality home care services to the patients. Universal Aides work under the supervision of the Home Care RN and Health Care Coordinator (RN) and under the indirect supervision of the Director of Activities. The Universal Aide will provide and assist participants of EveryAge PACE with activities of daily living, restorative, and supportive care. Assists with organization and implementation of activity program. Monitors health status and provides a safe environment for participants. Functions as a member of the Interdisciplinary Team.

Requirements

  • High school diploma or equivalent/ Certified Nursing Assistant (C.N.A).
  • Requirements of C.N.A are current certification as Certified Nursing Assistant in the State of North Carolina required.
  • Cardiopulmonary Resuscitation (CPR)/First Aid certification required.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Must have a valid driver’s license, proof of insurance and have means of transportation. Must be able to meet Carolina SeniorCare's auto insurance carrier's standards for coverage.
  • Successful completion of Certified Nursing Assistant program with current certification and required in-services.
  • A minimum of one year of experience in working with the frail or elderly population required.

Responsibilities

  • Make patient visits to perform health care services, documents patient activities and appearance, and reinforces patients' and their families' appropriate health care techniques and principals to enhance patient recuperation and/or development of patient skills in self-care under Care Plan and Registered Nurse or Therapy Director.
  • Carries out physician-approved care plans and nursing orders, as well as, prepares pertinent paper and chart work in order to provide effective, professional home care to patients and their families in compliance with federal, state, and local guidelines. Follows the written patient care instructions as prepared by the registered nurse.
  • Attends and participates in regularly scheduled meetings to discuss patient care, develop existing skills, or to learn new health care techniques and practices. Attends a minimum of twelve hours of in-service instruction per calendar year
  • Performs all duties relative to the Agency's definition of a standard visit.
  • Assists participants and/or supervises with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care, and toileting) as assigned or needed.
  • Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar and as requested by the Director of Activities.
  • Other duties as assigned by supervisor
