Requirements

High School diploma or GED required.
Minimum six (6) months of supervised experience as a Nurse Aide in a skilled nursing or acute care setting (one year preferred).
Must have completed a state‑approved Certified Nursing Assistant program with current certification.
Must be at least 18 years old preferred; candidates ages 16–17 may be limited per Department of Labor regulations and cannot operate power‑driven hoisting equipment.
Active listing with no substantiated findings on the state Nurse Aide Registry.
May require a written or manual competency test for Level 1 In‑Home Aide duties.
Must complete a minimum of 12 documented in‑service hours annually.
Must possess a valid driver’s license, proof of insurance, reliable transportation, and meet EveryAge auto insurance requirements.

Responsibilities

Provide patient home visits to deliver assigned health care services, document care, and reinforce appropriate health techniques with patients and families as outlined in the Care Plan.
Follow physician‑approved care plans and nursing instructions; complete required documentation and charting in compliance with federal, state, and local guidelines.
Maintain ongoing communication with the Home Care Agency Director RN, Clinical Supervisor, or Registered Nurses regarding treatment plans and patient needs.
Participate in scheduled meetings and complete a minimum of 12 hours of in‑service training per calendar year.
Observe and report changes in patient condition to supervisory clinical staff.
Follow the assigned weekly schedule to ensure visit quota requirements are met.
Perform all duties within the Agency’s definition of a standard visit and assist with additional duties as assigned.
