Requirements

  • High School diploma or GED required.
  • Minimum six (6) months of supervised experience as a Nurse Aide in a skilled nursing or acute care setting (one year preferred).
  • Must have completed a state‑approved Certified Nursing Assistant program with current certification.
  • Must be at least 18 years old preferred; candidates ages 16–17 may be limited per Department of Labor regulations and cannot operate power‑driven hoisting equipment.
  • Active listing with no substantiated findings on the state Nurse Aide Registry.
  • May require a written or manual competency test for Level 1 In‑Home Aide duties.
  • Must complete a minimum of 12 documented in‑service hours annually.
  • Must possess a valid driver’s license, proof of insurance, reliable transportation, and meet EveryAge auto insurance requirements.

Responsibilities

  • Provide patient home visits to deliver assigned health care services, document care, and reinforce appropriate health techniques with patients and families as outlined in the Care Plan.
  • Follow physician‑approved care plans and nursing instructions; complete required documentation and charting in compliance with federal, state, and local guidelines.
  • Maintain ongoing communication with the Home Care Agency Director RN, Clinical Supervisor, or Registered Nurses regarding treatment plans and patient needs.
  • Participate in scheduled meetings and complete a minimum of 12 hours of in‑service training per calendar year.
  • Observe and report changes in patient condition to supervisory clinical staff.
  • Follow the assigned weekly schedule to ensure visit quota requirements are met.
  • Perform all duties within the Agency’s definition of a standard visit and assist with additional duties as assigned.
