About The Position

Working outside of a medical facility, you’ll have an opportunity to get to know patients as you guide their care in the comfort of their homes. Monday - Friday daylight shifts. As a Home Care Aide with UPMC Home Healthcare, you’ll collaborate with our team of specialized nurses, rehabilitation therapists, social workers, dieticians, aides, and trained administrative staff to provide personalized patient care in a home setting. Requirements

  • High School Diploma or GED - Required
  • Minimum of 75 hours of Nurse Aide training -Required. If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.
  • Automotive Insurance
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Driver's License
  • Act 33
  • Act 34
  • OAPSA

Nice To Haves

  • One (1) year experience - Preferred
  • Certified Nurse Aide -Preferred

Responsibilities

  • Must maintain annual competency evaluation scores as determined by regulatory requirements
  • Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
  • Proven ability to facilitate change.
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
  • Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.
  • Participates in all required agency meetings.
  • Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient?s condition to appropriate professional staff.
  • Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
  • Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.
