Provides personal care and housekeeping assistance to clients in their homes; follows the established plan of care under the direction of the client care coordinator.
Requirements
- Home Care Aide or CNA Certificate approved by the Connecticut Department of Public Health and Addiction Services
Nice To Haves
- High school diploma or equivalent
- Six months experience as a Home Care Aide or CNA in a home care environment.
Responsibilities
- Provides personal hygiene and groom care to clients.
- Provides meals for clients’ nutritional needs.
- Reports changes in clients’ condition to client care coordinator or supervisor.
- Performs routine housekeeping and home management duties.
- Records each client’s care on care plan and submits care plan in a timely manner.
- Assists in providing emotional support for the client and family.
- Assists with routine therapy exercises.
- Reminds client of medication regime, opens medicine containers for clients as needed.
- Participates in case conferences as requested.
- Attends required in-service sessions.
- Will actively participate in the performance improvement process known as H3W.
