About The Position

The Public Health Home Care Aid provides direct personal care and homemaker services in clients’ homes while supporting public health programs, including WIC services, client admissions, and office operations. This position serves as a front-line representative of the agency, delivering compassionate care, assisting with intake and documentation, and supporting coordinated public health services. The Home Care Aide provides personal care and homemaker services in the patient’s home. Provides direct care to our patients in their residence that includes daily tasks of living such as bathing, and dressing. This position my also include some light housekeeping duties such as laundry, vacuuming, dusting, meal preparation and grocery shopping as well as other errands.

Requirements

  • Must have access to a reliable vehicle with current insurance coverage, and a state driver's license.
  • At least 18 years of age.
  • High school diploma or GED.
  • Successful completion of BCLS within 3 months of employment.

Nice To Haves

  • Certified Nursing Assistant certificate preferred

Responsibilities

  • Provides hands-on personal care and assistance with activities of daily living according to the plan of care and state regulations.
  • Observes, documents, and reports changes in client condition, behavior, and needs to appropriate nursing or supervisory staff.
  • Offers emotional support, encouragement, and promotes client independence and safety.
  • Performs light housekeeping tasks including laundry, meal preparation, shopping, and maintaining a safe, clean home environment.
  • Assists with care of clothing, linens, and bedding as required by the service plan.
  • Completes assigned household tasks in accordance with agency policies and client needs.
  • Provides general information and client assistance under the direction of public health program staff.
  • Maintains confidentiality and accurate records related to program services.
  • Assists with new client admissions including intake paperwork, data entry, scheduling, and coordination with nursing staff.
  • Collects, verifies, and processes client information and required documentation per agency policy.
  • Communicates admission status and client needs appropriate public health staff.
  • Maintains accurate service documentation, time, and mileage records.
  • Supports general office operations and assists public health program as needed.
  • Provides front-line client support including scheduling, data entry, filing, income and residency verification, and assisting with program intake.
  • Explains program benefits and assists participants with applications and required documentation in a health clinic setting.
  • Supports program goals such as breastfeeding promotion and referrals to community services while maintaining confidentiality and excellent customer service.
