About The Position

UPMC Family Hospice is hiring a casual Home Care Aide for the Lycoming, Union, Northumberland and surrounding areas Hours: 16 hours per period Weekend Rotation every 3rd-4th weekend and 2 holidays per year Provide designated care under the direction of the primary service provider in the home to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home.

Requirements

  • High School Diploma or GED - Required
  • Direct patient personal care in the last 5 years
  • Minimum of 75 hours of Nurse Aide training- Required. If no previous nurse aide training, the applicant may attend the UPMC Home Healthcare Nurse Aide Training Course as available upon hire.
  • Automotive Insurance
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Driver's License

Nice To Haves

  • One (1) year of experience - Preferred
  • Certified Nurse Aide- Preferred

Responsibilities

  • Direct patient care to the patients in the home setting
  • Effective communication to patient, family, and team members
  • Collaboration with interdisciplinary team to ensure a high level of patient safety and customer satisfaction
  • timely documentation within the patient's home
  • Must maintain annual competency evaluation scores as determined by regulatory requirements
  • Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures.
  • Proven ability to facilitate change.
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
  • Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization.
  • Participates in all required agency meetings.
  • Communicates appropriately with the interdisciplinary home care team promptly, reporting any changes in the patient's condition to the appropriate professional staff.
  • Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
  • Provides safe and direct personal care to patients in the home setting under the direct supervision of an appropriate professional.
